Practice statement


Please note the difference between medical coverage and vision coverage. IE: If you are booking an appointment for a wellness check, an infection, or injury, please list your medical insurance. If you are scheduling an appointment for new eyeglasses and/or contact lens exam, please list your vision insurance. It is often under a different name, even though the medical coverage is the primary carrier. Please have this information when you book your appointment as we will not be responsible to know your plan / benefits ahead of time. Please be aware that it is your responsibility as the member to know the specifics of your plan. Thank you!
